- A urine sample checked for specific gravity, protein, blood and sugar.
Ample Urgent & Primary Care offers DOT physical appointments at 10 Princeton Street in West Hartford. Whether this is your first exam or a renewal, call to confirm National Registry examiner availability and the records to bring.
Certification depends on the exam and required documentation. Plan ahead of your expiration date.
A DOT physical, sometimes called a CDL physical, checks whether a commercial driver meets the applicable medical standards for safe driving. For a federal DOT exam, the examiner must be listed on the Federal Motor Carrier Safety Administration's National Registry. The medical certificate is separate from your driver's license.
If you meet the standards, the examiner completes a Medical Examiner's Certificate, Form MCSA-5876. It can be valid for up to 24 months. A shorter certificate may be needed to monitor a condition, and some standards require more frequent exams.
FMCSA: DOT medical exams and certificate duration
Requirements depend on the vehicle, your work and whether you drive interstate or intrastate. Many non-excepted interstate drivers need medical certification. Some commercial drivers who do not need a CDL still need a medical exam. Ask which requirements apply to your work.
Your employer or Connecticut DMV can explain the rules and self-certification category for your driving. You must also report that driving category to the licensing agency; the medical exam is a separate requirement.

The examiner reviews your health history and medications, then completes the required testing and physical examination.
Tell the examiner what has changed since your last exam. The urine check is part of the physical. An employer's DOT drug test is separate.

When you call, mention any condition that may need outside records. Bringing them to the exam may save a second trip. The examiner may still need more information after seeing you.
For sleep apnea, ask whether to bring recent treatment notes and a CPAP usage report if you use CPAP. For heart disease, a recent procedure or another ongoing condition, ask which specialist records or test results the examiner wants.
If you use insulin for diabetes, the clinician who manages and prescribes your insulin must complete Form MCSA-5870. The medical examiner must receive the form and begin the exam within 45 calendar days after it is signed. Ask about the required glucose records when arranging that evaluation.

Ask the office which forms to complete in advance and whether your employer requires its own paperwork. Continue prescribed treatment unless your treating clinician tells you otherwise. Do not stop or change medication just to prepare for an exam.
Provide your license details, complete the health history and discuss medicines and records with the examiner.
The examiner performs the required checks and considers whether any additional documentation or evaluation is needed.
Ask about the decision, how long the certificate is valid, any restrictions and whether you need follow-up.
If you meet the standards and the needed information is available, the examiner can complete certification. If a decision is pending, records are missing or you do not meet the standards, ask what follow-up is needed. A shorter certificate is not a way to bypass a disqualifying condition.
Visit length varies. Allow time for check-in and paperwork, and schedule before your renewal deadline. An appointment request, a pending decision or a follow-up visit does not extend an expired medical certificate.
For Connecticut CDL and CLP holders and applicants, medical results reach DMV electronically through the National Registry. CT DMV does not accept a paper medical certificate from these drivers as the submission method. You can request a copy for your own records and use DMV's medical-status check to confirm receipt.
As of October 12, 2026, Connecticut-licensed CDL and CLP drivers and their employers cannot rely on a paper certificate as proof of medical certification. Completing the exam does not guarantee the state record updates immediately. Confirm your electronic status and any remaining licensing requirements before driving.
Medical examiners must report completed exams to FMCSA by midnight, local time, of the next calendar day. That reporting deadline is different from a promise that DMV will post your record the same day.

Track the date on your certificate and arrange the next exam early enough for outside records or follow-up. Connecticut DMV warns that an expired certificate can lead to a CDL downgrade or CLP cancellation. If a record has not updated or a license action has already occurred, contact DMV to find out what you need to do.
Non-CDL drivers may follow a different submission process. CT DMV directs non-CDL holders who must keep a medical certificate on file to its medical-certification portal. Follow DMV's instructions for your license type, and continue to carry any required waiver documents.
Call for the current DOT physical fee and ask what it includes. Do not assume a work-related certification exam is covered as an annual preventive physical. Ample's acceptance of an insurance plan does not establish coverage for this particular service.
Check with your insurer and employer before the visit. If your employer plans to pay, ask what authorization or billing instructions to bring. Confirm any separate charges for additional tests, outside evaluations or follow-up before they are arranged.
Review Ample's insurance and payment informationAmple provides primary and urgent care at the same West Hartford office. If the exam finds a health concern, you can arrange follow-up here or with your usual clinician. Treatment and DOT certification are separate: the examiner must still determine whether you meet the driving standards.
Dr. Mojdeh Heidari, DNP, FNP-C, is Ample's founder and a family nurse practitioner. Ask who will perform your DOT exam and confirm that examiner's current National Registry listing. Learn about our provider and primary care services, or read our school and DOT physical preparation guide.
No. The urine check in the medical exam looks for findings such as sugar, blood and protein. DOT drug and alcohol testing is a separate process. If your employer requires a drug test, ask what is needed and where it must be arranged; do not assume it is included in the physical.
We are open Saturday from 9:00 AM to 3:00 PM and closed Sunday. Call ahead to check DOT examiner availability and the appointment and paperwork requirements.
The time needed depends on your health history, the exam and any outside records or testing. Call about scheduling and allow time for paperwork. Certification may need more than one visit.
A diagnosis or prescription does not by itself answer whether you qualify. The examiner reviews the applicable standards, your treatment, symptoms and records. Insulin-treated diabetes has additional documentation requirements. Bring your medication list and ask which records are needed before the visit.
Do not assume your plan covers an employment or certification exam just because it covers an annual physical. Call for the current exam fee, check your benefits and ask your employer about payment arrangements. Additional tests or follow-up may have separate costs.
For a federal DOT medical exam, the examiner must be listed on the FMCSA National Registry. Being your usual clinician is not enough by itself. Ask who will perform your exam and confirm their current Registry listing.
A completed exam or a paper copy alone does not establish that your Connecticut CDL or CLP medical record is current. Confirm your electronic status and any other licensing requirements with DMV and your employer before driving. An expired certificate is not extended by an appointment request or a pending exam.
